Insurance & Medicare

Personal Nutrition does not accept private insurance or Medicare, but we will gladly provide you with a "superbill" so that you may submit a claim to your insurance company for reimbursement. Contact your insurer directly for information about the claims process and for specific information about benefits under your plan.

Our Policies...

1. Payment is due at the time of your appointment. We accept Cash or Check, and you may choose to use a credit card using Paypal on our website. Please make checks payable to Personal Nutrition, Inc. A $30 fee will be charged for all returned checks.
2. Cancellations must be made by phone 24 hours before the scheduled appointment so that we can schedule other clients for this time. No-shows or cancellations NOT made within the 24 hour period will be billed at $50.
3. Overdue payments must be paid before future appointments are scheduled. Late fees of 25% will be charged after 60 days. At this time, we will send your account to collections.
4. We provide our clients with the guidance and support necessary to meet personal goals; However, we believe strongly in client-driven changes and we are not responsible for lack of progress or the inability to meet goals.
5. The last 5 minutes of each appointment is used for scheduling and business matters.
