What to expect...

Generally, your first visit with a nutrition therapist involves a comprehensive nutritional assessment that takes about 1� hours. During this time, we will obtain your weight, height, and other measurements as indicated to complete the assessment. We will review your history, your current eating challenges, and what is important to you. In addition, we will evaluate your nutritional status, set realistic goals to ensure your success, and establish a plan of action. We want you to be successful, and we want your plan to be realistic for you and your family. It is recommended that you schedule at least one 30-minute follow-up appointment to evaluate your progress and work through challenges. Packages, which offer a significant cost-savings, are also available to achieve optimal results.


How to prepare...

Prior to your appointment, you will need to complete the "Client History" form which can be accessed on our website. To access this form, simply click on "Forms" and then "Client History Form." Print the form, complete it, and save to give to your nutrition therapist during your initial visit. It is recommended that you schedule an appointment when you can give your full attention to the session. Distractions may interfere with progress. In order to continue to offer the convenience of home visits, we must adhere to scheduled times. Thank you for being considerate and ready to start on time. We look forward to working with you!


Our Policies...

  1. Payment is due at the beginning of all sessions. Cash or Check is accepted. Please make checks payable to Personal Nutrition, Inc. A $30 fee will be charged for all returned checks.
  2. Cancellations must be made by phone 24 hours before the scheduled appointment so that we can schedule other clients for this time. No-shows or cancellations NOT made within the 24 hour period will be billed at the full rate.
  3. Overdue payments must be paid before future appointments are scheduled. Late fees of 25% will be charged after 60 days. At this time, we will send your account to collections.
  4. Some insurance companies cover services from a Registered Dietitian. We will provide you with a receipt called a "superbill" so that you can submit it to your insurance company for reimbursement.
  5. Packages must be used within 9 months of purchase, and no rebates or refunds will be given.
  6. We reserve the right to choose alternate meeting locations if safety concerns are identified.
  7. We encourage clients to allow us to communicate with primary care physicians. If you would prefer us NOT to contact your physician, please let us know.
  8. It is the mission of Personal Nutrition, Inc. to aid clients in meeting personal goals; however, the company is not responsible for lack of progress or inability to meet those goals.
  9. The last 5 minutes of each appointment is used for scheduling and business matters.
